Copy of the employer's order on compensation for the caused damage in the form according to Annexes No. 5 or No. 5-1 to the Resolution of the Cabinet of Ministers No. 177 of 24.06.2009, depending on the consequences of the accident at work;
Copies of the employee's or beneficiary's application containing a claim for compensation for the harm caused, addressed to the employer;
Certificates on the average monthly or annual salary of the employee prior to the insured event;
Copies of the employment agreement (contract);
In case of occupational illness - medical report on occupational illness of the employee;
Documents confirming the fact of occurrence of an industrial accident and its consequences for the employee (act of special investigation of an industrial accident, conclusion of a medical and labor expert commission, and, in case of a dispute, a court decision);
In case of the employee's death, a copy of the employee's death certificate and documents confirming the beneficiary's right to indemnification shall also be enclosed
