Documents for insurance compensation
- Insurance claim (in writing, within 30 days).
- In case of inpatient treatment: original medical record, and/or a copy of the inpatient medical record certified by the medical institution
- In the case of outpatient treatment: the original extract from the outpatient card, and/or a copy of the outpatient card certified by the medical institution.
- In injuries (fractures): X-rays
- All documents are submitted within 3 months of the event occurrence.
